Privacy Policy

What we collect

We collect the information you provide when you submit the free audit form: your name, work email, clinic name, and clinic URL. We also collect standard server logs (IP address, browser type, referring URL) via Cloudflare.

What we don't collect

We do not collect, receive, process, or store any patient health information (PHI). Our audit intake is explicitly designed to exclude PHI. If you inadvertently include PHI in a form submission, please contact us immediately at security@authoritize.ai and we will delete it.

How we use your information

We use your contact information to deliver your audit results and, if you request it, to follow up about our services. We do not sell your information to third parties. We do not share your information with third parties except as required to deliver our services (see our sub-processor list at /security).

Data retention

We retain audit submission data for 24 months. You may request deletion at any time by emailing hello@authoritize.ai.
